The FCC Form 481 is required for all ETCs participating in the High Cost and/or Lifeline programs. It is due July 1, 2014.
Carriers seeking USF support in either the Lifeline/Tribal Linkup or High Cost must submit the financial and operations information pursuant to the FCC form 481 and any information specific to the state (various states require filing in addition to the FCC) by July 1, 2014.

She will also have the assistance of our internal: Wholesale and CLEC; Wireless and Lifeline; IXC and CABS, consultants, to enable our clients to successfully manage current and future demands of the industry.
Samantha received her Bachelors of Science, with a minor in Economics, from the University of North Georgia, (The Military College of Georgia) and went on to acquire her BBA in Accounting, with a minor in Finance as well. Her experience and certification as a CMI – Sales & Use Tax from the Institute of Professionals in Taxation, display her dedication to understanding her area of expertise, and the industry has responded to her.
Samantha is routinely invited to speak, with her most recent presentations at CompTel, Telestrategies, and RIA user conferences on important issues like Federal USF rules, Managing Telecommunications Exemptions, and Transaction Tax Outsourcing. These are particular areas of foundational success within GSA. Samantha is the Foundations Committee Chair for the UNG Young Alumni Board and is an active member of (WICT) Women in Cable Telecommunications and (IPT) Institute for Professionals in Taxations. Samantha will be leading the Telecommunications tax instruction, for the upcoming 2015 session, at Georgia Tech. She will be leading on behalf of the IPT.

FCC released late 4/2/14 The Wireline Competition Bureau’s announcement of the final Lifeline Biennial Audit Plan. ETCs that receive $5 million or more from the low-income program, as determined on a holding company basis including all operating companies and affiliates, for calendar year 2013 will be subject to the first round of biennial audits.
